How to register for the Patient Access app

Step 1

Pick up an Online Booking Registration Form from any Indigo surgery.
Complete it and return it to our Reception Team, with proof of your identity.
Confirm you have read our Practice Policy for Online Appointment Booking and complete the Patient Agreement to Practice Policy for the use of Online Booking.
You will then receive login details, which you'll need for step 3, below.

Step 2

Download the app from the Apple or Android store.

Step 3

Create your Patient Access account with your information and the login details you received.

Step 4

Verify your email address and log in to your account. You will need to use the code sent to you by your surgery when you registered for Online Booking Registration. These codes may have different names depending on your GP practice:

Linkage key, Passphrase or PIN
Account ID or Access ID
ODS code or Practice ID

Please note these codes are case-sensitive.

Step 5

Click on Complete Registration.
